What you will need to complete this form

• Payment of course fees. After you have completed this enrolment form you will be directed to our payment system. If you do not pay the course fees, your enrolment will not be processed, and you will need to complete the form again.
• Sponsorship. If your course fees are being paid by a sponsor and your sponsor is not able to pay these fees now, using this form, then please contact us by email at admissions@gbmc.ac.uk , providing the name of the course and course code (above). Do not continue with this form because you will not be able to enrol without making a payment.
• Evidence to support reduced fee. You may be eligible to a 50% discount on the tuition fee if you have a gross annual salary below £22,308 or are in receipt of one of the following benefits:
  • Job seekers allowance
  • Employment Support Allowance
  • Universal Credit (and have a take home pay of less than £617 per month)
  • Income Support (and have a take home pay of less than £617 per month)
  • Council Tax Benefit (and have a take home pay of less than £617 per month)
  • Housing Benefit (and have a take home pay of less than £617 per month)
  • Pension Credit (and have a take home pay of less than £617 per month)
